test script: MySQL Server info is red

Hi - I'm trying to figure out if this will run on my server. I uploaded the test script and the MySQL Server info box is red, and it's asking me for a username and password (which is kind of sketching me out). Do I need to put this in to know for sure that the script will run or can I just ignore that?

Welcome to the forums,
Please create a database and after you do this you should create username and password and grant all privileges to this user on that created database. After that you should click a button to check connection to your database
